New Patients:

Please plan to arrive at our office 20 minutes prior to your appointment time to complete necessary paperwork.
You must bring your completed New Patient Packet that was mailed to you to your appointment. Failing to do so will result in your appointment being rescheduled.
Please make sure we know if this is a workers compensation claim or a motor vehicle accident.
Photo ID.
Insurance card.
Complete list of all medications.
All of your pain medications.
If your Primary Care Provider (PCP) has changed please let us know.

Please be courteous, we require 24 hours notification for all cancellations, this allows us time to bring in patients on our waiting list.
